Gramercy Specialty Clinic is committed to providing affordable care and it provides a multitude of payment options to cover the cost of services.  We currently accept a number of insurance plans, and also provide out-of-network services.  Our staff works directly with the insurance companies as a courtesy to our clients, and are happy to provide cost estimations regarding copay/coinsurance fees upon request. If you would like more information about your direct costs for services, please call our clinic and our staff will be happy to provide you with cost estimates, as well as match you with a health care provider that best meets your needs! Please know that our office only provides an "estimate of benefits". The actual costs of your services will be identified after the claims are paid by your insurance carrier. Please note that a $35 no show/same day cancellation fee applies to clients who fail to attend their sessions without notifying our office at least 24 hrs before their scheduled appointment time.


We currently accept the following insurance plans: 

Superior, Traditional Medicaid, CHIP, Star Health Foster Care, Blue Cross Blue Shield -Medicaid, Magellan, Ambetter (CORE plan, but not the Value plan), United Optum, Dell Childrens Health Plan, Blue Cross Blue Shield PPO & HMO, Humana, Tricare, Amerigroup and Aetna. 


Please note that clients who have Medicaid as well as an additional health insurance plan in place are required to provide both insurance plan information to prevent unexpected fees and invoices. Please notify our office each time your change your insurance plan so that we may inform you of your estimate of benefits under your new plan.

COUNSELING FORMS

All counseling services are rendered via Masters Level clinicians who hold a variety of training experiences and educational backgrounds. Our providers are trained in different areas of practice and all maintain one of the following clinical licensures: Licensed Professional Counselors (LPC), Licensed Clinical Social Workers (LCSW), Licensed Professional Counselor Associates (LPC Associate), Licensed Marriage & Family Therapist (LMFT) and Licensed Master Social Worker (LMSW).   Clients who do not have insurance may obtain services with one of our fully licensed clinicians (e.g. LPC, LCSW, LMFT) at a rate of $125 per session, as these licensures can practice independently.  These represent flat fees and are due at the time of service, and they are consistent regardless of the time spent in session.  A $35 no show/same day cancellation fee applies to clients who fail to attend their sessions without notifying our office at least 24 hrs before their scheduled appointment time.

The following forms are standard intake documents that contain information about our clinic's policies, procedures and confidentiality statement. Please feel free to complete these forms and bring them with you to your first appointment. Our staff will be happy to review them with you and answer any questions that you may have. If unable to print these forms, you will need to arrive 20 minutes early so that may complete them before your first appointment begins.

 No Insurance? No problem.

We offer a low-cost sliding fee scale for clients who do not have insurance or who are seeking a low cost, cash pay option. Clients who do not have insurance may opt for a affordable option of meeting with an LPC Associate or LMSW via our sliding fee scale, which ranges from $60-$100 per session, as these licensures typically work under the supervision and license of a Board-Approved Supervisor.  The cost for the initial session is $60 and then the subsequent fees will be determined based on the information provided on the sliding fee scale, and the follow up fees will be between $60 and $100 per session. These sliding scale fees are "per service" and are not related to a specific session length. The therapist will determine the session length based on the clinical needs of the client.
